Buttocks augmentation is commonly accomplished using one of two techniques; silicone implants or autologous fat transfer. Traditionally buttocks augmentation was accomplished with the use of silicone implants. Although often effective, a number a problems can be associated with this procedure. In the United States, the DEA restricts surgeons to only use solid silicone devices for buttocks augmentation as opposed to the much softer silicone gel devices now available for breast augmentation. Due to the increased firmness of these devices, patients often complain of an unnatural feel and discomfort associated with these solid implants.
In my practice I have found fat transfer to be a wonderful method for buttocks augmentation. In this procedure a generous amount of fat is harvested from areas of excess such as the abdomen and thighs. The fat is prepared on the operating table and transferred to the carefully pre-operatively chosen areas of the buttocks to improve contour and/or overall volume. Patients tolerate the procedure well and can often return to most of their normal daily activities in a number of days.
Successful buttock augmentation by fat grafting method depends on careful preoperative assessment and planning. Most patients benefit not only from what is added to the buttocks, but also from what is taken away from the surrounding areas of the lower back, flanks and hips by liposuction.
